PACE (Program of All-Inclusive Care for the Elderly) provides an all-inclusive approach
to healthcare that includes primary and specialty care, acute care, long term care,
transportation, prescription medications, durable medical equipment, therapies, case
management, in-home personal care and chore services, and an adult day program. Care
is coordinated by a team of professionals to assist adults age 55 and older remain
independent in their housing of choice.
