The VNA offers diverse programs to support elders’ specialized health needs and to enable elders to stay in their homes as long as possible. Services include:

  • Geriatric Care Management which helps elders and their families maximize independence, safety and quality of life through consultation, assessment and care planning with expertise from nurses and social workers.
  • Adult Day Programs in Colchester and Williston are for adults unable to remain at home alone.  Our South Burlington (Grand Way) ADP is dedicated to persons with dementia.  All programs offer nursing, personal care services, enrichment and activities appropriate to the individual.  Support groups for persons with dementia and for caregivers are offered at the Grand Way site.
  • VNA Private Care/Care Connection provides a variety of private in-home services such as personal care, housekeeping, meal preparation, errands, companionship and respite.
  • A Homemaker Program serves low-income frail elders and adults with disabilities.
  • A Foot Care Program and wellness care services are offered at various Senior Residential Living facilities throughout Chittenden County.
  • Adult Home Care provides high-quality care for people of all ages with acute and long-term illnesses.  In addition to skilled nursing services, specialty nurses coordinate individualized care.  Licensed nursing assistants help with personal care and activities of daily living.  Physical, occupational and speech therapists aid clients in their rehabilitation.  Social workers link people with supportive services.  End-of-Life Care programs include Hospice, Palliative Care and Vermont Respite House.
